To avoid disappointment, please call the Centre to confirm vacancies prior to completing the Enrolment Form. The form must be printed out, completed and returned to the centre with original signatures, accompanied by the enrolment fee and your child's original birth certificate and immunisation records.
In addition to the enrolment form, please download and complete the
Parent Permission Form as well as the appropriate age group form below.
0-2 years Supplementary Form - Click to download
This form contains additional information for children birth to 2 years, and must be printed out and completed for all children within this age range and returned to the Centre upon commencement.
2-5 years Supplementary Form - Click to download
This form contains additional information for children 2 to 5 years, and must be printed out and completed for all children within this age range and returned to the Centre upon commencement.
Waitlist Form - Click to download
Complete and return this form to be placed on Dinky Di Children's Learning Centre's waiting list for care.
